If this is your first visit, please bring

- List of past surgeries (including date of your surgery) and medical problems
- Any test results pertaining to your current issues
Please arrive 15 minutes early for the appointment.
We encourage you to invite your family members or anybody whom you would like to come to your appointment. We are a small clinic, please limit your companions to 2-3 people.

Billing
We accept payment by cash, check, debit, credit card, CareCredit card, flexible spending account (FSA) and health savings account (HSA). Major insurance companies are accepted.
We recommend that patients contact their insurance company to verify their benefits prior to their appointment.
We are happy to bill your insurance for a hearing or balance evaluation, if we are in-network. Some insurance companies have hearing aid benefits. We will help you in determining benefits that can assist with the cost of the hearing aids. For any sound sensitivity appointments, we do not claim to your insurance. It is out of pocket.
Most insurance plans may not cover the following procedures. We collect a non-refundable fee out of pocket for these services:
- Earwax removal
- Earmold impressions
- Earplugs
- Hearing aid services on hearing aids not directly purchased from PAC Audiology
- Hearing aid consultation
- Tinnitus treatment consultation and treatment follow-ups
- Hyperacusis treatment consultation and treatment follow-ups
- Misophonia treatment consultation and treatment follow-ups
If correct information is not provided at the time of service, you will be responsible for all charges as billed and an additional $25 rebilling fee.

Directions to Bellevue clinic
I-405 Northbound:
Take the NE 4th St. exit eastbound. Turn right at the first light (NE 4th ST), move to the left lane, and turn left at the next light (116th. Ave. NE). Stay on this road for 1 mile. Turn right into the driveway located in front of the Eastview Building sign. Follow the blue sign that says 1370. We are in the back building, Eastview.
I-405 Southbound:
Take the NE 8th St. exit eastbound. Move to the left lane and turn left at the first light (116th. Ave. NE). Stay on this road for 1.5 miles. Turn right into the driveway located in front of the Eastview Building sign. Follow the blue sign that says 1370. We are in the back building, Eastview.
Parking is available free of charge.
There are 4 handicapped parking spaces on the west side of the building. There are 4 guest 2-hour parking spaces in the basement of the building.
