What to Do if Your Hearing Suddenly Drops in One Ear

Noticing that one ear suddenly sounds muffled can be unsettling. Sometimes the cause is simple, like a buildup of earwax. Other times it's sudden sensorineural hearing loss (SSHL), a condition that needs medical care right away. Knowing the difference, and knowing who to call first, gives you the best chance of protecting your hearing.
Sudden Hearing Loss in One Ear Is a Medical Emergency
If your hearing drops in one ear over a few hours or a few days, treat it as an emergency. Go to an urgent care clinic or see an ear, nose, and throat physician (ENT) as soon as possible, ideally the same day.
SSHL happens when something affects the inner ear or the nerve pathways that carry sound to the brain. It usually affects one ear, and many people first notice it when they wake up. Some also experience ringing in the ear, a feeling of fullness, or dizziness.
Timing matters because treatment tends to work best when it starts early. Physicians often prescribe corticosteroids to reduce inflammation and swelling in the inner ear. The National Institute on Deafness and Other Communication Disorders reports that about half of people with sudden hearing loss recover some or all of their hearing on their own, and that delays in diagnosis and treatment can make treatment less effective. Please seek care quickly, even if the change seems mild.
How to Tell It Apart from an Earwax Blockage
Earwax is one of the most common reasons an ear suddenly feels plugged. Wax usually builds up gradually, although a blockage can seem to appear all at once, often after showering, swimming, or using a cotton swab that pushes wax deeper into the canal.
A few details can point toward earwax:
- The change started after water got in your ear or after cleaning it with a swab
- Your ear feels itchy, full, or plugged, usually without dizziness
- You've had wax buildup before
Other details suggest something more serious:
- Your hearing dropped quickly with no clear trigger
- New ringing, roaring, or dizziness came with the change
- You heard a loud “pop” just before your hearing changed
The challenge is that the two can feel very similar, and there's no reliable way to tell them apart at home. A clinician can look inside your ear with an otoscope and quickly see whether wax is blocking the canal. If the canal is clear, simple in-office tests can help show whether the problem lies in the inner ear. That's why urgent care or an ENT should be your first stop. In the meantime, avoid cotton swabs, ear candles, or other objects, which can push wax deeper or injure the ear canal.
Why a Comprehensive Hearing Test Comes Next
Once you've seen a physician and started any recommended treatment, the next step is a comprehensive hearing evaluation with an audiologist. This appointment uses a variety of tests to measure how well each ear hears across different pitches and how clearly you understand speech. The results give you and your physician a detailed picture of where your hearing stands.
That first evaluation also serves as a baseline. Follow-up testing can then show whether your hearing is recovering, holding steady, or needs further attention.
At PAC Audiology, we receive many diagnostic referrals from physicians and work closely with them for each patient. Our medical reports present the most important findings clearly, which helps your doctor plan your care. If an exam shows that wax is the cause of your muffled hearing, we also offer earwax removal in Bellevue so you can get back to hearing comfortably.
What Happens if Some Hearing Doesn't Return
Even with fast treatment, some people have lasting hearing loss or tinnitus in the affected ear. Hearing well from only one side can make it harder to tell where sounds are coming from and to follow conversations in noisy places.
There are good options for managing these changes. Our audiologists take time to listen to your concerns, go over your results in detail, and talk through treatment options such as hearing aids and tinnitus management. Family members are welcome to join your appointment.
